This is a heartwarming opportunity for compassionate providers who love making a difference! We work with members throughout Wisconsin to find providers that fit each of their needs. Our goal is to help individuals be in more control of their own supports and to have a say in who is assisting them. This helps them to stay in their own homes as long as possible.

Job Title: Private Duty Nurse (PDN)
Job Location: Clintonville, WI (in-home)

WAGE:
• LPN: $29.75/ hr.
• RN: $44.63/ hr.

SCHEDULE:
• To be officially discussed and determined between provider and managing party. 
• Seeking the following day shifts:
o Monday – Friday - occasional: 8am-8pm
• Seeking the following night shifts: 
o Monday – Thursday: 8pm-8am
o Sunday: 8pm-8am

JOB DUTIES:
• Provide care in client’s home. 
• Comfortable with transfers utilizing a ceiling lift.
• Comfortable with providing safe and effective tracheostomy care; including suctioning and ventilator. 
• Assist with Activities of Daily Living (ADLs) to include toileting, bathing, dressing, and administering medications. 
• Ability to engage with clients while providing safety and companionship.

QUALIFICATIONS:
• Driver’s license and reliable transportation. 
• CPR and ventilation certification.
• Must be at least 18 years of age at the start of employment.
• Must pass background check
